1/28/2024 EnchantaVaughn Approved Behavior: During the first part of his therapy, JaCaeri was seen on task and exhibiting a good attitude. JaCaeri expressed positive thoughts to his counselor and seemed to be focused and alert during the role play activity. JaCaeri continues to be disrespectful. Behavior: JaCaeri was observed making good choices and avoiding negative behaviors. JaCaeri appeared to be in an ecstatic mood, as evidenced by his enthusiasm and positive expressions. JaCaeri was seen displaying inappropriate social interactions during the session. Intervention: QMHP-C facilitated activity to allow JaCaeri insight into several conflict resolution and problem-solving skills to utilize at home. QMHP-C encouraged JaCaeri to role play appropriate and inappropriate conflict resolution skills in efforts to highlight and demonstrate several appropriate options JaCaeri may utilize when he feels challenged or the need for impulsive disruptive actions. QMHP-C provided strategies such as positive reframing. QMHP-C assisted JaCaeri with practicing reframing negative thoughts with more positive thoughts in efforts to reduce impulsive reactions to situations that usually trigger JaCaeri's negative expressions. JaCaeri was able to reduce physical overactivity and become more attentive in session. QMHP-C discussed with the family one goal they believe they need to work on individually and as a family. Intervention: QMHP-C had each member write down goals and discussed through brainstorming and creating a goals chart on how to achieve each one. QMHP-C prompted JaCaeri to express how he believes his family should treat each other. QMHP-C encouraged JaCaeri to think twice before reacting negatively or aggressively in situations that may not need that level of reaction to be solved. QMHP-C used the basic listening sequence to clearly draw out the point of view that JaCaeri was feeling and get beyond his aggressive language and behaviors. QMHP-C explored positive behavior options he was exhibiting with JaCaeri to support his willingness to continue to make good decisions and display appropriate behaviors in the home. QMHP-C processed thoughts and feelings with parent. QMHP-C encouraged JaCaeri to continue expressing himself and letting go of his stress and anxiety. Response: JaCaeri understands that problem solving starts with thinking before acting. JaCaeri demonstrates problem solving by staying calm and using his senses. JaCaeri acknowledges expectations and reviews goals and objectives with counselor and parent in order to get familiar with how he is supposed to react to different conflicts in the home. JaCaeri displays good social behavior, as seen sharing and taking turns. JaCaeri implemented the positive reinforcement provided by his counselor and was able to stay motivated and enjoy his day. JaCaeri exhibited good manners after earning incentive for his hard work. JaCaeri focused on things he is good at and loves to do at home in order to stay motivated in the home/community setting and make something out of himself. JaCaeri did a good job ignoring distractions and maintaining appropriate behavior in the home. JaCaeri administered positive behavioral expectations, reducing ADHD. As JaCaeri has made more of his own decisions, he reported increased satisfaction and function. JaCaeri responded well to feedback, interacting well with his peers. JaCaeri agreed to behave and communicated a positive attitude when reviewing problem solving and self-control strategies. Response: JaCaeri promised he would try his best to correct himself when faced with conflict. JaCaeri was displaying respectful, non-aggressive behaviors in the home. JaCaeri expressed positive thoughts about himself. JaCaeri stated he didn't need help with anything at the moment but would tell an adult if he needs help. JaCaeri communicated by nodding and exhibiting good eye contact. JaCaeri verbalized positive feelings about goals and objectives and implementing them daily.
